During your first appointment with one of the specialists at Care Fertility we will need you to provide the following information:

• past medical and surgical histories of both partners
• family history
• current medications
• any past treatments for infertility or which may have influenced fertility of both partners
• if you have copies of past investigations that have been performed please bring them with you to your consultation
• write down the questions you wish to have answered.

Remember to allow plenty of time before your appointment in case there are delays related to traffic, parking or finding the location of your fertility specialist.
